Step-by-Step Instructions

1. I accidentally deleted a note

Scribematic can restore lost notes for up to 30 days from the time of recording.

To restore a note:

  1. Open the Scribematic.AI website.

  2. Click the chat bot in the bottom right corner and request a restoration.

  3. Or, email [email protected] with the name and date of the note.


2. I can't see my notes across devices

Ensure that you are logged in with the same email on all devices. Using multiple accounts may prevent notes from syncing.


3. My recording/note is stuck loading

Try these steps:

  1. Click the Recordings tab.

  2. Refresh your browser page.

  3. If these steps don't work, click the chat bot in the bottom right corner or email [email protected] with the name and date of the note.


4. My microphone isn't working

To enable your microphone:

  1. Go to Settings → Privacy & Security → Microphone.

  2. Ensure the app or website (Scribematic) has permission to access your microphone.

  3. Confirm the correct microphone is selected if you have multiple devices.


5. My recording stops early

Sometimes recordings end early due to device settings such as a screen saver, auto-lock, or power-saving feature.

To fix this:

  1. Check your device for any screen saver or auto-lock settings and disable them while recording.

  2. Or keep your device active during the recording session.

  3. If you need instructions for adjusting these settings, contact Scribematic Support via [email protected] or use the Chat Bot at Scribematic.AI.


6. After finalizing my note, an "error" message appears

  1. Refinalize or reprocess your note.

  2. If the issue persists after three attempts, contact Scribematic Support.


7. After finalizing my note, it doesn't look how I want it

  • Try re-finalizing or reprocessing the note.

  • Review the note for any missed context or errors before saving.


8. General Tips When Having Issues

  • Refresh your browser to ensure the latest version of Scribematic is loaded.

  • Clear cookies: Go to Menu Bar → History → Show Full History → Delete Browsing Data, then select Cookies.

  • Clear cache: Follow the same steps but select Cache instead.

2. How do I Manage Moving Back and Forth Between Patients with Scribematic?

Overview

  • Edits made through Talk to Your Note modify the note text only

  • If the note is refinalized later, Talk to Your Note edits need to be reapplied


Step-by-Step Instructions

How to Pause and Resume a Recording

  1. Start recording for Patient A as usual.

  2. When you need to step away, tap the microphone icon to pause the recording.

  3. Begin a new recording for Patient B.

  4. When you return to Patient A:

    • Open their recording from your list.

    • Tap the microphone icon again.

    • Scribematic will continue recording into the same note.

There's no need to merge files — Scribematic automatically appends the new audio to the original recording.


How to Unfinalize a Note

  1. Open the note.

  2. Tap Unfinalize at the top of the screen.

  3. Resume recording as needed.

This allows you to add additional dictation before locking the note again.


How to Edit After the Visit with "Talk to Your Note"

Use Talk to Your Note to apply edits directly to the note text.

Important Details:

  • Edits made through Talk to Your Note modify the note itself

  • They do not change the original audio recording

  • If you refinalize later, you'll need to reapply any Talk to Your Note edits

Best Practice Tip: If you anticipate additional updates, consider unfinalizing first before making major changes.

4. How Do I Create and Use Templates in Scribematic?

Understand Template Types

Exam, Review of Systems, and Procedure Templates

These templates are designed to be used within Checkbox Finalize. When finalizing a note with Checkbox Finalize, you will see dropdown menus where your Exam, ROS, and Procedure templates will be available. Many clinicians can copy their existing EHR macros directly into Scribematic.

My Complete Note Templates

These templates provide maximum control over Scribematic's output. When Finalize from Template is selected, the template defines the full structure of the note and instructs the AI on how each section should be written. This is ideal for clinicians who want their notes to closely follow a specific documentation format.


Create an Exam Template

  1. Navigate to the Templates tab.

  2. Select Exam at the top left of the template home page.

  3. View the sample templates to get familiar with the format.

  4. Select Create Template.

  5. Name your template and add content (you can copy and paste an existing exam template from your EHR).

  6. Save to your template library.

  7. To use this template: select Checkbox Finalize → Objective → Exam → Select the template from the dropdown.

  8. Test out your new template and make any adjustments needed.


Create a "My Complete Note" Template

Scribematic provides two primary options:

Option 1: Build Your Own Template

Go to Templates → My Complete Note → Create Template → Build Your Own

  • Name the template

  • Add sections: Section headers, Verbatims, or Instructions

  • Your newly generated template will appear inside your template library

  • To use: Select Finalize from Template and choose your template from the dropdown

Option 2: Create from EHR Note

  • Select Create Template → Create From EHR Note

  • Name your template

  • Copy and paste a completed note from your EHR into the box

  • Select Generate Template

  • Your newly generated template will appear in your library for review and editing

  • To use: Select Finalize from Template and choose your template from the dropdown


Use the Sample Template Library

  • Select Sample Template Library (middle of template home page)

  • Browse templates by specialty

  • Save a template to your account

  • Customize using the Edit button

  • To use: Select Finalize from Template and choose your template from the dropdown


Share Templates (Enterprise Users)

Providers within the same Enterprise Group can share templates. Many organizations choose to standardize documentation by creating templates that are used across the entire practice. These templates can be preloaded into individual user accounts.

  • Revisit templates during your first few weeks of use

  • Be specific when writing instructions for the AI

  • Include guidance about what information or behavior should be avoided

AI Data Usage & Model Training

Patient Data Is Not Used to Train AI Models

Patient visits, recordings, and generated notes are not used to train AI models. Scribematic does not allow the AI to learn from or retrain itself on patient encounters.

Improving Output Without Using Patient Data:

  • Templates — Providers can instruct the AI exactly how notes should be structured

  • Custom Lexicon — Define commonly used terms, medications, and phrases for consistent documentation

  • "How Could This Note Be Improved" Feedback — Sends feedback directly to the Scribematic engineering team without using patient data

Access Control: Only authorized users can access clinical data through secure authentication and controlled user permissions.


Data Retention & Deletion

Recording Retention:

  • Recordings are encrypted and temporarily stored for provider review

  • Recordings are deleted from provider accounts after 30 days

Final Deletion:

  • After removal from accounts, data is held for an additional secure 30 days before permanent deletion


Clinician Responsibilities for Compliance

Providers Should:

  • Obtain appropriate patient consent when required

  • Review and verify AI-generated documentation before finalizing notes

Once you click Stop Recording, finalize options will appear at the bottom of the note.


1. Quick Finalize

Best for: Simple, fast notes with minimal customization.

Quick Finalize:

  • Automatically analyzes the encounter

  • Determines relevant sections

  • Generates a complete SOAP note

  • Requires minimal input

Use this when:

  • You want speed and simplicity

  • The encounter follows a standard outpatient structure

  • You do not need highly customized formatting


2. Checkbox Finalize

Best for: Quick notes with more structural control.

Checkbox Finalize allows you to:

  • Select specific Subjective sections

  • Select specific Objective sections

  • Control which components are included

Note: Selections are held between finalizations. If you have a standard set of required fields, you will not need to adjust in Checkbox Finalize every visit — only open the dropdown to make changes.

History of Present Illness options: Multi-complaint or Extensive Assessment and Plan options: Multiple formats available

Use this when:

  • You routinely document specific exam elements

  • You want to standardize note structure

  • You prefer guided customization rather than full templating


3. Finalize from Template

Best for: Full customization and specialty-specific workflows.

Allows you to apply a "My Complete Note" template to your recording. Templates allow you to:

  • Define exact structure and section order

  • Insert specialty-specific instructions

  • Mirror your documentation style

  • Add structured prompts or formatting rules

Use this when:

  • You want full control over structure

  • You practice in a specialty with specific documentation standards

  • You want consistency across all notes


4. Dictation Finalize

Best for: Physicians who prefer post-visit dictation-style workflows.

Designed for situations where you are:

  • Speaking directly into the microphone after the visit

  • Reading structured information

  • Creating documentation outside of a patient conversation

Includes multiple formatting options. The "My Complete Note" option lets you select a template you have created for your dictation format.

Use this when:

  • You prefer traditional dictation workflows

  • You are documenting after a visit rather than recording during a visit


5. Inpatient Finalize

Best for: Rounding, handoffs, and multi-clinician discussions.

Designed for:

  • Hospital rounding

  • Care transitions

  • Conversations involving multiple clinicians

  • Longitudinal or continuity-based documentation

Select Clinical Discussion when conducting an extensive discussion of the patient's situation. Works well for rounding. Combines subjective/objective findings, history, etc. into a relevant clinical discussion.

Select Systems Based Assessment and Plan when an extensive list of systems is discussed (e.g., cardiovascular, gastro) and the current system status and plan are mentioned.

Use this when:

  • You are documenting inpatient care

  • Continuity and context across encounters matter

  • The conversation involves multiple providers

Step-by-Step Instructions

Custom Lexicon

Purpose: Improve documentation accuracy for commonly misinterpreted terms.

Go to Account → Settings → Custom Lexicon

Custom Lexicon allows you to define:

  • Provider names — Ensure the AI spells your name correctly

  • Medication names — If Scribematic misspells a medication, add it to your Custom Lexicon

  • Practice-specific terminology

Once added, Scribematic will consistently document those terms correctly in future notes.


Language Settings

Purpose: Control the output language of your notes.

Go to Account → Settings → Language Settings

Scribematic:

  • Automatically detects the language spoken during a visit

  • Does not require manual input settings for language detection

  • Allows you to choose your preferred note output language

Live transcription is currently available in English.


AI Model Selection

Purpose: Adjust performance preferences.

Go to Account → Settings → Select AI Model

Scribematic uses multiple AI models to generate notes. By default, the Automatic Router selects the most appropriate model. You can manually select models if you prefer:

  • Faster processing

  • More detailed formatting

  • Specific output behaviors

Most users benefit from leaving the automatic model selected.


Auto Finalize

Purpose: Automate your workflow.

Go to Account → Note Settings → Auto Finalize

Auto Finalize automatically completes a note as soon as a recording ends using your most recently selected finalize option.

This works best when:

  • You use the same finalize method consistently

  • You want to reduce clicks

  • You prefer a streamlined workflow

Note: If your note is Auto-finalized and you weren't ready, you can Unfinalize the note by scrolling to the top of the note.


Copy Forward

Purpose: Maintain continuity across encounters.

Copy Forward allows you to start a new note using a previous note as context. Especially useful for:

  • Inpatient care

  • Rounding

  • Follow-up visits

  • Longitudinal care documentation

How to use:

  1. Select the note from the patient's previous visit.

  2. Once finalized, look for the copy forward icon (box with arrow) in the top right corner.

  3. Select Copy Forward, name your new note, and tap the microphone to begin the new visit.

The new note will include information from both the previous and current visit.

Important: Scribematic only stores records for 30 days, so Copy Forward will only work on visits within 30 days of each other.


EHR Integrations

Go to Account → Integration → Manage Integration

Integrations allow for:

  • Streamlined workflows

  • Reduced manual copying and pasting

8. How do I Create My First Note in Scribematic?

Overview

This article provides a high-level overview of Scribematic's workflow and guides you through creating your first medical note. Scribematic is an ambient listening medical scribe designed to capture the full provider–patient encounter and transform it into a structured, professional medical document using AI.


Best Practices and Tips

  • Choose the device (cell phone, tablet, laptop) that best supports your workflow. Many providers record on a phone or tablet and then edit and upload to EHR later on desktop.

  • Scribematic is fully web-based and accessible on desktop, tablet, or mobile devices.

  • Some providers find that starting the recording pre-visit with a quick intro (patient's name, age, gender, and reason for visit) reduces post-visit edits.

  • Introduce yourself and anyone else in the room so Scribematic knows who to listen for.

  • Narrate your exam — briefly describe what you're doing and call out key findings as you go. Scribematic can't see what you're doing; it can only hear what you say.

  • Feel free to be as personable as you like with your patients. Scribematic will only include what is medically relevant in the finalized note.


Step-by-Step: Creating Your First Note

1. Log In

Navigate to scribematic.ai and log into your account.

2. Start a Recording

  1. Click the Recordings tab at the top of your screen.

  2. Select the large blue Start button.

  3. Enter the patient's name or initials for your note when prompted.

  4. Click blue Record to begin.

You may now conduct your patient encounter as you normally would. Scribematic will capture the full visit in the background.

Note: First-time users may need to allow browser permissions to access their microphone.

3. Stop the Recording

  1. Click the blue Record button again to stop recording.

  2. You will be prompted to select a finalization method.


Choosing a Finalization Option

For first-time users, we recommend:

  • Quick Finalize – for a fast and reliable SOAP note

  • Checkbox Finalize – offers structured control while remaining streamlined

In most introductory workflows, Quick Finalize is recommended. (See the article on Finalization Options for a full comparison.)


Reviewing Your Generated Note

Once finalized, Scribematic generates a completed medical document. Before moving the note into your permanent records:

  1. Carefully review the document.

  2. Use Scribematic's editing tools to make any necessary adjustments.

  3. Confirm accuracy and completeness.


Moving the Note to Your EHR

Scribematic is EHR-agnostic, meaning it is designed to work alongside any Electronic Health Record system.

To transfer your note:

  • Click the Copy button at the top right of individual note sections, or

  • Click "Copy All" at the top of the document to copy the full note at once

  • Paste the content directly into your patient chart within your EHR

  • Use names naturally during conversation

Example: "I'm Dr. Smith and this is our medical assistant, Sarah. Nice to meet you, Annie."

For Multilingual Visits:

  • Scribematic automatically detects the spoken language during the first portion of the encounter

  • Live transcription may not appear during non-English visits

  • The visit is still being captured and processed normally

  • Finalized notes will generate in your preferred output language


Step-by-Step Instructions

How to Use the Telehealth Audio Capture (Beta)

Step 1: Enable the Telehealth Audio Feature

  1. Go to your Scribematic Account page.

  2. Scroll down to the Beta Features section.

  3. Toggle on Enable Telehealth Audio Capture. When enabled, a Telehealth button appears during recording to capture system audio from video calls (Zoom, Teams, etc.) alongside your microphone.

Step 2: Start a Telehealth Recording

  1. Set up a new recording as you normally would. Select Start and name your recording.

  2. Tap the blue Microphone button. Once recording begins, look for a black bubble labeled Telehealth under the blue button.

  3. Click the Telehealth button to enable audio capture.

Step 3: Choose What to Share
